Most personal tax advice is written for people whose income is well below yours. Once you are earning enough, the familiar moves have already phased out, and the planning that remains is different in kind rather than degree.
What has already gone
By the time a Fresno physician, attorney or executive household is well into six figures, several things people still talk about are simply unavailable:
- The QBI deduction, if you are in a specified service trade or business. Health, law, accounting, consulting and similar fields lose it entirely above $494,600 of taxable income for joint filers, or $247,300 otherwise for 2025.
- Direct Roth IRA contributions, above the income limits.
- Deductible traditional IRA contributions, where covered by a workplace plan.
- Various credits that phase out well below your income.
Knowing what is gone matters, because a lot of energy gets spent chasing deductions that stopped applying years ago.
What still works
Retirement plan capacity
This remains the largest lever, and if you own the practice or firm, capacity is a design decision rather than a fixed limit. For 2026 the elective deferral is $24,500, catch-up at 50 and over is $8,000, ages 60 to 63 get $11,250, and total annual additions per participant reach $72,000.
Beyond that, a cash balance plan layered over a 401(k) can push deductible contributions substantially higher, with capacity increasing as you age. For an owner in their fifties with stable income, this is usually the single biggest remaining deduction.
Timing, across years rather than within one
At high income the useful question is which year, not whether. Bunching deductions into alternating years so they clear the standard deduction threshold, accelerating income into a year you know will be lower, deferring into a year you know will be higher — these require a multi-year view rather than a December scramble.
Charitable strategy, done properly
If you give anyway, how you give matters more than how much at this income level:
- Appreciated securities held long term, rather than cash. You deduct the value without recognising the gain.
- Donor advised funds to concentrate several years of giving into one deduction year while distributing over time.
- Qualified charitable distributions from an IRA once eligible, which satisfy required distributions without adding to adjusted gross income.
Investment location
Which account holds which asset is a genuine tax decision. Holding tax-inefficient assets in tax-deferred accounts and tax-efficient ones in taxable accounts compounds quietly over decades. So does harvesting losses systematically rather than in a panic each December.
Real estate
Rental property brings depreciation, and for those who qualify, the real estate professional rules change how losses can be used. The qualification bar is high and genuinely tested, so it is worth an honest assessment rather than an optimistic one.
The California layer
California has no preferential rate for long-term capital gains — they are taxed as ordinary income at state level. That changes the arithmetic on holding periods, on where you realise gains, and on charitable gifts of appreciated assets, in ways that national advice does not account for.
Coordination is where the value actually is
At this income level the individual moves matter less than whether they fit together. Your entity structure affects your retirement plan capacity. Your retirement plan affects your taxable income. Your taxable income affects whether charitable timing helps. Your investment decisions affect all of it.
Handled separately by people who do not talk to each other, these work against one another regularly. The most common pattern we see is a well-run practice, a reasonable investment portfolio and a competent tax return, with nobody looking at the three together.
That is the gap worth closing, and it is closed in October, not April.
